ABSTRACT

Profound changes have taken place in human disease spectrum, constitution spectrum, and drug use behavior, and the safety of traditional Chinese medicine(TCM) faces new trends and problems. In particular, serious adverse reactions/events such as liver injury and kidney injury caused by non-toxic TCM have been frequently reported, overturning people's understanding of TCM safety, and even shaking the public's confidence in the development of TCM. In the new era of globalization, correctly understanding the situation and problems of TCM safety and addressing the dilemmas in safety evaluation and risk prevention of TCM are the key missions to be undertaken by TCM practitioners. This paper suggests that the situation and problems of TCM safety should be viewed objectively and dialectically, and the use standard of TCM should be advanced with the times. Furthermore, this paper puts forward the new conception and methodology of TCM safety(including one innovative understanding, two types of evaluation modes, tri-elements injury hypothesis; four-quadrant risk decision processes, and five-grade safety evidence body) for the first time, hoping to provide new theories, new strategies, new methods and successful examples for solving the safety problems of TCM.

ABSTRACT

Amid the modernization and internationalization of traditional Chinese medicine(TCM), the safety of TCM has attracted much attention. At the moment, the government, scientific research teams, and pharmaceutical enterprises have made great efforts to explore methods and techniques for clinical safety evaluation of TCM. Although considerable achievements have been made, there are still many problems, such as the non-standard terms of adverse reactions of TCM, unclear evaluation indicators, unreasonable judgment methods, lack of evaluation models, out-of-date evaluation standards, and unsound reporting systems. Therefore, it is urgent to further deepen the research mode and method of clinical safety evaluation of TCM. Based on the current national requirements for the life-cycle management of drugs, this study focused on the problems in the five dimensions of clinical safety evaluation of TCM, including normative terms, evaluation modes, judgment methods, evaluation standards, and reporting systems, and proposed suggestions on the development of a life-cycle clinical safety evaluation method that conformed to the characteristics of TCM, hoping to provide a reference for future research.

ABSTRACT

Since the safety re-evaluation of traditional Chinese medicine(TCM) injections began in 2009, some TCM injection companies and research institutes have done a lot of work. And with the increase of drug development and drug production technology levels in China, the safety of some TCM injections has been greatly improved. There are safety risks in TCM injections, which are mainly reflected in unclear basis of medicinal materials, simple production process, poor controllability of quality standards, nonstan-dard drug instructions and irrational medication in the use process. This paper describes the research progress of the above-mentioned aspects of TCM injections. In addition, the author team found that adverse reactions of TCM injections are mainly pseudo-allergic reactions. Therefore, a lot of work has been done in detection of pseudo-allergic reactions, mechanism research and risk control. This part of the work is also described in this article.

ABSTRACT

We report on a 51-yr-old woman who developed intravascular hemolytic anemia caused by arsenic after long-term ingestion of a traditional Chinese medicine (TCM). Twelve years before the admission, she was diagnosed as neurocysticercosis. She has ingested a TCM for about 12 yr instead of undergoing medical therapy for the disease. She was presented with a severe Coombs'-negative hemolytic anemia with hemosiderinuria. The urine arsenic level was elevated suggesting the arsenic intoxication as a cause of the anemia. She was treated successfully with therapeutic red cell exchange without any sequelae.

ABSTRACT

Se realizó un estudio descriptivo y analizó el tratamiento con acupuntura y sus técnicas a pacientes con asma bronquial en el municipio de Fomento, Las Villas, en el período de enero a diciembre de 1998, con un total de 200 pacientes pertenecientes a diferentes grupos etáreos, los que constituyeron el 100 (por ciento) de nuestra muestra. Se estudiaron algunos aspectos como la relación existente entre el asma bronquial y algunos factores biopsicosociales como: edad, sexo, ocupación. Se analizó la evolución con la acupuntura, auriculoterapia, moxibustión, ventosa, etc. En nuestro estudio se demostró la gran utilidad de estos sencillos y económicos procederes para el control y buena evolución del paciente con asma bronquial, lo cual contribuye a reafirmar los verdaderos objetivos y premisas de la Directiva 26, y de esta forma estimular el uso de la acupuntura y sus técnicas en nuestro municipio, para contribuir a un mejor desarrollo de nuestra medicina. Se concluyó que se obtuvo una buena evolución con todas las técnicas empleadas(AU)


A descriptive study was made and the use of acupuncture and its techniques to treat patients with bronchial asthma in the municipality of Fomento, Las Villas, from January to December, 1998 was analyzed. The sample was composed of 200 patients divided into different age groups. The relationship between bronchial asthma and some biosocial factors as age, sex and occupation was among the studied aspects. The evolution of patients with acupuncture, auriculotherapy, moxibustion, ventouse, etc. was analyzed. In our study, it was demonstrated the usefulness of theses simple and economic procedures for the control and good evolution of the patient with bronchial asthma, which contributes to reaffirm the real objectives and premises of Directive 26, and encourages the use of acupuncture and its techniques in our municipality to promote the development of our medicine. A good evolution was observed with all the techniques(AU)
